Access to reliable, affordable healthcare remains one of the most critical challenges in developing nations. While capital cities often attract telemedicine startups and digital health investments, regional districts and tier-2 cities are frequently left navigating fragmented, pen-and-paper systems.
Patients in district headquarters like Feni Sadar (Bangladesh) regularly struggle with:
- Finding verified, BMDC-registered specialist doctors and their updated chamber visiting hours.
- Unpredictable, out-of-pocket diagnostic lab expenses.
- Lack of transparent emergency ambulance contacts during critical golden hours.
To bridge this gap, we engineered and launched Health Club Feni—a modern, full-stack digital healthcare directory and membership network designed specifically for the local ecosystem.
(Note: Despite the name, Health Club is strictly a medical care directory and membership platform, not an athletic fitness gym).
In this article, I want to break down the architectural choices, engineering constraints, performance optimizations, and search engine strategies we used to build this platform for real-world patients and healthcare providers.
🛠️ The Tech Stack
When designing for regional users who often browse on budget Android smartphones and fluctuating 3G/4G connections, every millisecond and kilobyte counts.
- Framework: Next.js 16 (App Router) with React 19
- Rendering Architecture: React Server Components (RSC) first, with strict client boundary isolation
- Database & Auth: Supabase (PostgreSQL) paired with Prisma ORM
- Styling: Tailwind CSS v4 + Radix UI / shadcn accessible primitives
- Offline & Service Worker: Modern PWA caching via Serwist
- Form State & Validation: React Hook Form + Zod
- SEO / AEO Engine: Generative Engine Optimization (GEO), Schema.org
MedicalWebPage&Physicianentity graphs, IndexNow integration
🏛️ Core Architectural Challenges & Solutions
1. High Performance via Pure React Server Components (RSC)
Rather than loading heavy client-side JavaScript bundles, over 85% of Health Club's public pages are rendered as pure Server Components.
When a patient visits the Feni Specialist Doctor Directory or the Diagnostic Tests & Investigations Catalog, the server delivers pre-rendered semantic HTML with zero hydration overhead:
// Server Component: Instant database query with zero client JS bundle bloat import prisma from "@/lib/prisma"; import { DoctorCard } from "@/components/doctors/DoctorCard"; interface Props { searchParams: Promise<{ department?: string; location?: string }>; } export default async function ConsultantsPage({ searchParams }: Props) { const { department, location } = await searchParams; const doctors = await prisma.doctor.findMany({ where: { status: "ACTIVE", ...(department && { department }), ...(location && { location }), }, include: { partner: true }, orderBy: { priorityOrder: "asc" }, }); return ( <section className="max-w-7xl mx-auto px-4 py-8"> <h1 className="text-3xl font-extrabold text-slate-900"> Specialist Doctor Roster in Feni Sadar </h1> <div className="grid grid-cols-1 md:grid-cols-2 lg:grid-cols-3 gap-6 mt-6"> {doctors.map((doctor) => ( <DoctorCard key={doctor.id} doctor={doctor} /> ))} </div> </section> ); } This guarantees near-perfect Google Core Web Vitals (LCP < 1.1s, CLS = 0) even on constrained mobile hardware.
2. Solving Diagnostic Affordability: The 10–30% Member Discount Network
In Bangladesh, high out-of-pocket medical expenditure pushes millions into financial vulnerability. Diagnostic investigations (blood work, ultrasound, digital X-rays, CT scans) often constitute the largest initial bill.
Health Club built a formal offline-to-online network with verified Partner Hospitals and Diagnostic Centers in Feni Sadar. Through the platform's Digital Membership Card, registered patients receive an official 10% to 30% discount on laboratory and radiology investigations simply by presenting their verified digital member ID at partner reception desks.
+-------------------------------------------------------------+ | Health Club Membership Flow | +-------------------------------------------------------------+ 1. Patient Registers Online (Free/Nominal Annual Sub) │ ▼ 2. Instant Dynamic QR & Member ID Card Issued (PWA-cached) │ ▼ 3. Patient visits Contracted Diagnostic Partner in Feni Sadar │ ▼ 4. Partner Scans/Verifies ID ──> 10-30% Instant Discount Applied Architectural rule: To maintain contract compliance and integrity, discounts are communicated as percentage benefits (10-30% Member Discount) while providing transparent market reference ranges rather than misleading fixed prices.
3. Emergency Triage: 24/7 Helplines & Symptom Navigator
Regional patients frequently hesitate during medical emergencies, unsure whether a symptom warrants immediate hospitalization or a scheduled specialist visit.
We built two key safety nets:
- 24/7 Feni Ambulance & Emergency Helplines: A verified, community-updated directory of oxygen suppliers, blood donor networks, and ambulance hotlines with direct one-tap mobile dialing.
- Clinical Symptom Triage Navigator: A lightweight clinical decision-tree based on WHO and DGHS emergency protocols, identifying critical "Red Flag" symptoms (e.g., crushing chest pain, sudden unilateral weakness, acute breathlessness) and redirecting users to the nearest emergency ward before booking standard outpatient appointments.
4. Generative Engine Optimization (GEO) & Machine-Readable Knowledge Bases
With users increasingly finding medical information through AI answer engines (ChatGPT, Perplexity, Gemini, Claude) alongside traditional Google search, we integrated state-of-the-art GEO (Generative Engine Optimization):
- Entity Disambiguation: Every doctor, partner clinic, and test is enriched with Schema.org JSON-LD (
@type: Physician,Hospital,MedicalWebPage). - Standardized Machine Knowledge Bases: We publish machine-readable markdown files at
/llms.txtand/llms-full.txtdirectly mapping all verified doctors, test guidelines, and partner facilities for LLM web crawlers (GPTBot,PerplexityBot,ClaudeBot). - Bilingual Intent Handling: In Bangladesh, queries alternate between conversational Bengali and English/Banglish keywords (e.g., "Feni doctor serial", "CT scan price in Feni"). Every entity satisfies both intents simultaneously.
📈 Key Engineering Takeaways
- Keep Client Boundaries Minimal: In Next.js App Router, resisting the temptation to slap
"use client"on top-level pages dramatically cuts bundle size and eliminates client-side hydration waterfalls. - Design for Low-Bandwidth Reality: Heavy single-page apps (SPAs) with 500KB+ vendor bundles fail real-world users in district towns. Pure RSC + responsive WebP/AVIF images deliver real accessibility.
- Structured Data is the Future of Local Search: Deep schema graphs and clear entity definitions allow both Googlebot and modern LLM answer engines to cite local services with high accuracy.
